PTC: positive temperature coefficient thermistor, unknown value,

D5: depends on the MOSFETs T4; with a 30V type such as SMP60N03-10L,
use ZF24; with 50 V or 60 V MOSFETs, use ZF43 or ZF51

cells; sets the undervoltage shut-off point)
